Consent form
A document where a participant ticks each use of their data, then signs and dates to agree to take part after reading the information statement. A guardian signs if the participant is a minor.
Understanding Consent form in VCE Data Analytics U3O2
What it does: A participant under 18 cannot give valid consent alone, so the consent form has a space for a parent or guardian to sign. The first tick box confirms the participant read the information statement before agreeing, so their consent is informed. Tick boxes for each use let people choose exactly how their data may be used, such as quotes in the report or sharing with other researchers. The date shows when agreement was given, so the form needs both the signature and the date.
Common mistakes: A person may only sign after reading the information statement. The line should say 'I read the information sheet.'. The participant is a minor, so a parent or guardian must sign. It should read 'Guardian signature: Kim Tran'. A friend cannot give consent.
